Oxford Handbook of Critical Care

Of all the medical specialties, few if any are as exacting and complex as critical care medicine. All members of the multidisciplinary team caring for critically ill patients require a sound knowledge of physiology, pathophysiology, biochemistry, technology and pharmacology.
The new edition of this enormously popular Oxford Handbook describes best practice in critical care in a succinct, concise, clinically orientated way. It covers therapeutic and monitoring devices, drugs and fluids, specific organ system disorders and complications, and general management philosophies. Ample space is provided to append or amend sections to suit local protocols and particular practices.
The Oxford Handbook of Critical Care will serve the consultant, junior doctor, nurse or other paramedical staff as a reference book, aide memoire and handy pocket book providing rationales and solutions to most of the problems encountered.

Emergency Medical Services:At the Crossroads

Emergency Medical Services (EMS) is a critical component of our nation s emergency and trauma care system, providing response and medical transport to millions of sick and injured Americans each year. At its best, EMS is a crucial link to survival in the chain of care, but within the last several years, complex problems facing the emergency care system have emerged. Press coverage has highlighted instances of slow EMS response times, ambulance diversions, trauma center closures, and ground and air medical crashes. This heightened public awareness of problems that have been building over time has underscored the need for a review of the U.S. emergency care system. Emergency Medical Services provides the first comprehensive study on this topic. This new book examines the operational structure of EMS by presenting an in-depth analysis of the current organization, delivery, and financing of these types of services and systems.

Miller's Anesthesia, 6th Edition

"Miller's Anesthesia is without a doubt a complete and thorough reference of current knowledge in the field of anesthesia. Its strenght lies in its use of a wide range of recognized authors, contributors, and consulting editors, as well as a plethora of scientific information and extremely well-referenced chapters. Miller's Anesthesia is a reference text that every clinician should own or, at the very least, have access to. There are other reference books that provide quick summaries of various topics, but they should not be a replacement for such an in-depth and complete text.
There are significant new chapters in the Sixth Edition. Some of these reflect the importance of new developments in medicine and society as they relate to anesthesia. Perhaps chief among these is the chapter about medical concerns associated with chemical and biologic warfare. Other new additions include chapters about implantable cardiac pulse generators, perioperative blindness, anesthesia for robotic surgery, and human performance in patient safety. It is worth noting that, by and large, the existing chapters from previous additions seem to have been updated with more recent information. In fact, some of these chapters are by new authors and have been completely revised.

US Army medical course - Blood, Electrolytes, and Intravenous Infusions

This course is concerned with knowledge that is vital in order to understand the "whys" and "hows" for using the intravenous route to administer the kinds of fluids that can save life and limb. You should possess the knowledge to correct and expand on the medical procedures used by the Combat Medical Specialist and also have the ability to interact with the physicians and physician assistants who will have the final responsibility for treating battle injuries and illnesses. Some of the terms used in this course may be unfamiliar to you. You will have frequent contact with the physician and physician assistants and will need to understand their vocabulary. It would be wise to use one of the excellent medical dictionaries available in most bookstores and libraries. Some examples are the latest editions of Dorland's Illustrated Medical Dictionary (published by W. B. Saunders Company) or Taber's Cyclopedia Medical Dictionary (published by F. A. Davis Company). For general English usage, an example is Webster's New Collegiate Dictionary (published by G. and C. Merriam Company). Do not feel restrained by these examples. Any good dictionary will be useful.

U S Army Medical Course:Anesthesia Apparatus

For over two thousand years, physicians of many cultures have performed
surgery upon anesthetized patients. The anesthetics ranged in type and effectiveness.
During certain periods of history, earlier anesthetics were forgotten. In the recent past, a physician relied upon the patient consuming alcohol and biting down hard on anything convenient. Modern general anesthesia began in 1846 when Dr. J.C. Warren first used ether. A dentist named Heidbrink invented the first anesthesia administering machine in1910. Aside from a few refinements, the machines physicians use today are verysimilar to the machine invented by Heidbrink. The Ohmeda 885A Anesthesia Apparatus is one such machine.
This course provide medical support to the combat soldier under field conditions. Most clinical medical equipment is not rugged enough to survive the field environment, so field medical equipment was designed. The Ohmeda 885A Anesthesia Apparatus is a field anesthesia unit which is rugged, portable, and self-contained. It is your job to keep it operationally efficient. The
material in this subcourse covers the preventive maintenance checks and services
(PMCS), routine maintenance, calibration and verification, and repair procedures you use to accomplish this task.

2005 American Heart Association Guidelines
For CPR And ECC

The American Heart Association (AHA) guidelines for cardiopulmonary resuscitation (CPR) and emergency cardiac care.1 These guidelines, based on the latest comprehensive review of resuscitation science by the International Liaison Committee on Resuscitation, emphasize streamlined algorithms and recommendations to reduce the amount of information rescuers must learn and to emphasize the most important skills they must master.
The guidelines are based on the work of eight task forces that addressed specific areas of resuscitation science: basic life support, advanced life support, acute coronary syndromes, life support for children, neonatal life support, stroke, first aid, and education. These groups used an evidence-based approach to classify the recommendations for CPR and emergency cardiac care. The guidelines also present 12 algorithms covering the essential assessments and interventions for cardiac arrest and other life-threatening conditions.

A Pocket Book of the European Resuscitation Council
Guidelines for Resuscitation

This pocket book contains the European Resuscitation Council Guidelines for Resuscitation 2005. It follows hard on the heels of the 2005 International Consensus Conference on Cardiopulmonary Resuscitation and Emergency Cardiovascular Care Science with Treatment Recommendations produced by
the International Liaison Committee on Resuscitation published in "Resuscitation" in November 2005. The European representatives at that conference, held in Dallas in January 2005, more than pulled their weight in the process of producing the Consensus on Science conclusions arising as a result of presentations and debate. Their names are listed at the end of this preface and the resuscitation community, in Europe and beyond, is most grateful to them for their talent, dedication and selfless hard work. In addition, they, and many others from Europe, also produced worksheets addressing the evidence for and against every conceivable detail of resuscitation theory and practice. The European Resuscitation Council (ERC) Guidelines contain recommendations that, by consensus from the European representatives, are suitable for European practice in the light of today's conclusions agreed in the Consensus on Science. As with the Consensus on Science document, they represent an enormous amount of work by many people who have worked against the clock to produce the Guidelines for Europe. Each section of the guidelines has been masterminded and coordinated by the leaders of the ERC working groups and areas of special interest. Such ventures do not happen without leadership and we are grateful to Vinay Nadkami, Bill Montgomery, Peter Morley, Mary Fran Hazinski, Arno Zaritsky and Jerry Nolan for guiding the Consensus on Science process through to completion. It would not be invidious to single out Jerry Nolan, the ILCOR Co-Chairman, for thanks and praise. He is universally respected and popular and has proved to be a wonderful ambassador for Europe. His scientific credibility and understanding is beyond doubt and his integrity, dedication, sheer hard work, patience and meticulous attention to detail and sensitivities have won the admiration of all. He has led the Consensus on Science process on our behalf and has been the lead coordinator in producing the European Guidelines.

Guidelines for essential trauma care
( WHO 2004 )

The authors of the guidelines have developed a series of resource tables for essential trauma care that detail the human and physical resources that should be in place to assure optimal care of the injured patient at the range of health facilities throughout the world, from rural health posts whose staff do not have training as doctors, to small hospitals staffed by general practitioners (known as GP-staffed hospitals), to hospitals staffed by specialists (specialist-staffed hospitals), to tertiary care centres. They also take into account the varying resource availability across the spectrum of low- and middle-income countries.2 Finally, a series of recommendations is made on methods to promote such standards including training, performance improvement, trauma team organization and hospital inspection. The resource tables and associated recommendations are intended to provide a template to assist individual countries in organizing and strengthening their own trauma treatment systems. It is anticipated that the template will be adapted to suit local circumstances.

Cardiac Arrest Lite! animated version

Cardiac Arrest! Lite is a graphically simpler version of the simulator -- either for users with older computers or for those who don't want all the noise and action of the video version. It includes all the drugs, all the patients, and all the fun.
This is state-of-the-art computer learning. Not "Choose A,B,C" nor "Guess what I'm thinking" but a real simulation that's stimulating and fun.
Patients cover a wide variety of medical problems associated with cardiovascular collapse. (And each patient behaves differently each time resuscitation is attempted.) Many patients pose difficult therapeutic priorities, presenting problems beyond heart attacks, such as hypocalcemia, hypothermia, and hyperkalemia.
Cardiac Arrest! casts you in the role of emergency physician. The patient's history is presented, and the resuscitation efforts begin. You read the ECG on the computer screen, interpret the vital signs and laboratory data, and give treatment orders. ACLS becomes a challenging computer adventure game.

Evidence-Based Management of Patients with Respiratory Failure

Respiratory failure is a complex disease process whereby the underlying disease and therapeutic measures interact. The patient’s outcome is determined by a variety of factors including how we use therapeutic maneuvers such as mechanical ventilation for prevention of complications, e.g., ventilator associated pneumonia. This book is a comprehensive review of all aspects of acute respiratory failure, providing critical reviews of various topics related to respiratory failure and discussing the clinical evidence available for the diagnoses and management of patients with respiratory failure. This book contains an extensive bibliographic review, focusing on preventive and therapeutic studies, that was methodologically standardized, with authors assessing and classifying studies according to statutes of evidence-based medicine. It considers epidemiology and outcome of mechanical ventilation; addresses ventilator modes and utility of pulmonary mechanics monitoring for treatment; analyzes physiologic effect and patient-outcomes of pulmonary recruitment and lung protective ventilation procedures; describes complications that can be present in these patients such as ventilator associated pneumonia and useful methods to prevent respiratory infections; covers the impact of bronchodilators, corticosteroids, and antibiotics in acute exacerbation of chronic obstructive pulmonary disease; discusses how, when, and in whom to do tracheostomy; and evaluates the use of sedation and neuromuscular blockade, as well as current clinical trials in acute lung injury. In summary, this book on the Evidence-based Management of Patients with Respiratory Failure provides important information to improve patient outcome by clearly identifying the research evidence that we can apply in daily clinical practice.

Hospital-based Emergency Care
At the Breaking Point (Future of Emergency Care)

Today our emergency care system faces an epidemic of crowded emergency departments, patients boarding in hallways waiting to be admitted, and daily ambulance diversions. Hospital-Based Emergency Care addresses the difficulty of balancing the roles of hospital-based emergency and trauma care not simply urgent and lifesaving care, but also safety net care for uninsured patients, public health surveillance, disaster preparation, and adjunct care in the face of increasing patient volume and limited resources. This new book considers the multiple aspects to the emergency care system in the United States by exploring its strengths, limitations, and future challenges. The wide range of issues covered includes:
The role and impact of the emergency department within the larger hospital and health care system.
Patient flow and information technology.
Workforce issues across multiple disciplines.
Patient safety and the quality and efficiency of emergency care services.
Basic, clinical, and health services research relevant to emergency care.
Special challenges of emergency care in rural settings.
Hospital-Based Emergency Care is one of three books in the Future of Emergency Care series. This book will be of particular interest to emergency care providers, professional organizations, and policy makers looking to address the deficiencies in emergency care systems.

Surgical Critical Care Vivas

An understanding of critical care is one of the key objectives of medical and surgical training, and viva examinations in particular remain a source of anxiety for many candidates. Surgical Critical Care Vivas follows the same format as the hugely popular General Pathology Vivas and gives candidates a means of practising some of the most common questions that they will be asked in their viva examinations. Pocket-sized, for portability, and packed with useful information in an east-to-use A-Z format, this book will help the reader to broaden their knowledge base and, by practising the vivas, gain confidence in their examination technique.

Tropical and Parasitic Infections in the Intensive Care Unit

In the past, many tropical and parasitic infections were confined to tropical areas of the world located between the Tropic of Cancer and the Tropic of Capricorn. However, with the increase in air travel and tourism and the changing patterns of immigration, an increasing number of individuals are coming into contact with these infectious agents and transmission across the world has been enhanced.
Tropical and Parasitic Infections in the Intensive Care Unit provides an international perspective on this topic and an overview of those infections that may cause critical illness.

Key Questions in Surgical Critical Care

An understanding of critical care is vital to the modern surgeon, who is often responsible for patients requiring intensive care or high-dependency support. Key Questions in Surgical Critical Care has been designed as a companion to Surgical Critical Care, by Robert Ashford and Neal Evans, and is split into two main sections, multiple choice questions and viva topics. Based on the syllabus of the Royal College of Surgeons, each of these two sections is further subdivided into six sub-sections covering the cardiovascular system, the respiratory system, other systems and multisystem failure, problems in intensive care, principles of intensive care and practical procedures. The MCQs and viva topics provided are typical of those appearing in the MRCS examination, and the authors provide detailed explanatory notes to accompany each answer, almost all of which are cross-referenced directly to the relevant pages of Surgical Critical Care, as well as other sources of further reading.

An Introduction to Clinical Emergency Medicine:
Guide for Practitioners in the Emergency Department

Winner of the 2006 AMWA Book Awards Physicians Category, An Introduction to Clinical Emergency Medicine is a much-needed resource for individuals practicing in this challenging field. It takes a novel approach, describing in detail the best and most current methods including initial patient evaluation, generation of differential diagnoses, problem-solving and management of challenging conditions based on presenting symptoms. Unlike other textbooks, in which the diagnosis is known, this textbook approaches clinical problems as clinicians approach patients - without full knowledge of the final diagnosis. It provides an understanding of how to approach patients with undifferentiated conditions, ask the right questions, gather historical data, utilize physical examination skills and order and interpret laboratory and radiographic tests. It provides current management and disposition strategies with controversies presented, including pearls and summary points for each topic covered. The book is multi-author, each contributor chosen because of a track record in teaching as well as being internationally recognised experts in the specialty.

Anaphylaxis (CIBA Foundation Symposia Series)

Anaphylaxis is an immediate-type allergic reaction involving the whole organism. It is the most life-threatening allergic condition. Although there are few exact epidemiological data regarding prevalence, estimates regarding insect sting anaphylaxis range from 1--3 per cent in the general population, but much higher values have been reported by some authors for food and drug-induced anaphylaxis. Anaphylaxis is the main acute killer of allergic individuals. Although anaphylaxis was discovered at the beginning of the 20th century, there are still many unresolved issues. These include non-IgE-mediated anaphylactoid reactions, non-immunologically mediated anaphylactoid (pseudo-allergic) reactions, pathophysiological events at the microcirculatory level, appropriate therapy for the acute reaction, strategies for prevention, public education about the problem and new approaches to prevention and therapy at the IgE level. All these subjects are discussed in this book. Since anaphylaxis occurs acutely and is unforeseen, it is very difficult to organize controlled studies regarding therapy and prevention. The spectrum of symptomatology covers many clinical areas (skin, respiratory, cardiovascular and gastrointestinal system), therefore inter-disciplinary approaches are necessary for progress in the field. There is widespread uncertainty among physicians about therapy, especially concerning self-administered treatment. In this important book, an multidisciplinary group of experts explore the pathophysiology of different types of anaphylactic and anaphylactoid reactions. Evidence is presented on the epidemiology of these conditions while problems relating to diagnosis, therapy and prevention are examined in detail. This thorough and up-to-date coverage of the subject will be of great interest to all clinical immunologists, researchers and physicians who deal with this life-threatening condition.
An interdisciplinary group of experts explore the pathophysiology of different types of anaphylactic and anaphylactoid reactions in this timely publication. Data are presented on the epidemiology of these conditions and problems concerning diagnosis, therapy and prevention are discussed. Cardiovascular and microcirculatory events are examined, including microcirculatory disturbances, anaphylaxis during anesthesia, and the role of histamine receptors in the response of the heart to anaphylaxis. There is detailed consideration of the different initiators of anaphylaxis, including food, radiographic media, insect venom, and artificial materials used in operating rooms. The book ends with a discussion of the various controversies surrounding diagnosis and therapy.
